paścātkarma: what the term means and how it is translated

paścātkarma पश्चात्कर्म is an Ayurvedic technical term. It is rendered into English in 4 different ways, which is the reason this page exists: the renderings do not agree, and the difference between them changes what the term is taken to mean.

How it is translated, and whether the translation holds

Each rendering is given with the source that uses it. The assessment is ours.

Posttherapy procedures

WHO International Standard Terminologies on Ayurveda (2022) · ITA-9.2.109 paścāt-karma: 'The variety of procedures which are advised after a major therapeutic procedure in order to return a person to normalcy.' Companion entries ITA-9.2.4 pūrvakarma 'Preparatory procedures' and ITA-9.2.68 pradhāna-karma 'Main therapeutic procedure'. · comparator-standard

WHO places paścāt-karma at the end of the cleansing-therapy section, after the śuddhi-grading entries, so it is scoped to panchakarma aftercare; the Sushruta source scopes it to all treatment including surgery.

Reliable; he keeps the Sanskrit in parentheses. His footnote offers two commentarial readings of what falls under each head.

The entire course of medical treatment in connection with a disease may be grouped under three subheads, as the Preliminary measures (Purva- karma ); the Principal therapeutical or surgical appliances (Pradhana-karma); and the After-measures (Paschat-karma). Bhishagratna's rendering 'After-measures (Paschat-karma)'; he scopes the triad to 'the entire course of medical treatment'.

Not to be confused with

pūrvakarma
The preparatory phase (Su.Su.5.3; WHO ITA-9.2.4); paścātkarma is the after phase of the same triad.
pradhānakarma
The main procedure (WHO ITA-9.2.68) that paścātkarma follows.
saṁsarjana krama
The graded diet after vamana/virecana; often described as the paścātkarma of those procedures (Bhishagratna's footnote places 'rice meal' under paścāt-karma), but it is one content of the phase, not its name.
parihāra kāla / parihārya
The period and the things to be avoided after a procedure (Ca.Si.1.4 asks 'किं वर्जनीयं प्रतिकर्मकाले, कृते कियान् वा परिहारकालः'); restrictions during paścātkarma rather than the phase itself.
paścāt (adverb) in other compounds
NIA 2011 has only 'mūtrayutaḥ pāk paścāt' (a symptom description); the therapeutic compound is absent there.

Renderings that do not hold

  • post-operative care (as the whole meaning). Correct for Shalya, but Sushruta scopes the triad to all treatment of disease and WHO scopes it to panchakarma aftercare.
  • follow-up (in the biomedical sense of review visits). Paścātkarma is an active regimen (diet, restrictions, further procedures), not monitoring.
  • after-treatment. Suggests a lesser treatment given afterwards; the phase is part of the one karma that the triad divides.

Open questions

Recorded rather than resolved. Each one is something this entry cannot currently settle.

  • Whether Charaka uses the compound paścātkarma; the Siddhisthana speaks of parihāra-kāla and saṁsarjana without this term in the lines read.
  • The commentary (Dalhana) behind Bhishagratna's footnote assignments was not consulted.
  • The edition behind siva.sh's verse numbering is unstated; wikisource's is Hari Sastri 1980.
