## How it is translated, and whether the translation holds

Each rendering is given with the source that uses it. The assessment is ours.

### Posttherapy procedures

WHO International Standard Terminologies on Ayurveda (2022) · ITA-9.2.109 paścāt-karma: 'The variety of procedures which are advised after a major therapeutic procedure in order to return a person to normalcy.' Companion entries ITA-9.2.4 pūrvakarma 'Preparatory procedures' and ITA-9.2.68 pradhāna-karma 'Main therapeutic procedure'. · comparator-standard

WHO places paścāt-karma at the end of the cleansing-therapy section, after the śuddhi-grading entries, so it is scoped to panchakarma aftercare; the Sushruta source scopes it to all treatment including surgery.

Reproduced unaltered and attributed from the WHO International Standard Terminologies on Ayurveda, which is what that

### the After-measures (Paschat-karma)

Bhishagratna, Sushruta Samhita vol. 1 (1907), Sutrasthana ch. V · wisdomlib doc122892 ('Chapter V - Preliminary measures') · public-domain-translation

Reliable; he keeps the Sanskrit in parentheses. His footnote offers two commentarial readings of what falls under each head.

Gloss only; edition behind its verse numbering unstated.

### post-operative care / post-procedure regimen / paschat karma

Common textbook and PubMed usage (search 'paschat karma ayurveda' returned 95 records, mostly describing panchakarma protocols; no abstract read uses the term as its subject) · textbook-usage · our paraphrase of the source, not its words

Usage is split between surgical aftercare and panchakarma aftercare (saṁsarjana krama).

## The classical passages

### Sushruta Samhita Sutrasthana 5 (Agropaharaṇīya):3 (sa.wikisource text of the Hari Sastri 1980 edition; siva.sh also displays ॥३॥), checked against the source

> त्रिविधं कर्म पूर्वकर्म प्रधानकर्म पश्चा / :त्कर्मेति तद्व्याधिं प्रत्युपदेक्ष्यामः ३

tr. Sanskrit text (no translator); the wikisource line break falls inside पश्चात्कर्म and is shown as ' / :' · https://sa.wikisource.org/wiki/सुश्रुतसंहिता/सूत्रस्थानम्/अध्याय_०१-१५

Karma is threefold: pūrvakarma, pradhānakarma, paścātkarma; these will be taught disease by disease.

### Sushruta Samhita Sutrasthana 5:3 (as displayed on siva.sh; edition unstated), checked against the source

> त्रिविधं कर्म- पूर्वकर्म, प्रधानकर्म, पश्चात्कर्मेति; तद्व्याधिं प्रत्युपदेक्ष्यामः ॥३॥

tr. Sanskrit text (no translator) · https://www.siva.sh/sushruta-samhita/sutra-sthana/5/3

Same sentence, second Sanskrit source with punctuation; both agree on verse 3.

### Sushruta Samhita Sutrasthana V (Preliminary measures):Unnumbered in Bhishagratna; Sanskrit 5.3 per the two sources above, checked against the source

> The entire course of medical treatment in connection with a disease may be grouped under three subheads, as the Preliminary measures (Purva- karma ); the Principal therapeutical or surgical appliances (Pradhana-karma); and the After-measures (Paschat-karma). These measures will be discussed under the head of each disease as we shall have occasion to deal with t

tr. Kaviraj Kunja Lal Bhishagratna (1907); quote truncated at page extraction boundary · https://www.wisdomlib.org/hinduism/book/sushruta-samhita-volume-1-sutrasthana/d/doc122892.html

Bhishagratna's rendering 'After-measures (Paschat-karma)'; he scopes the triad to 'the entire course of medical treatment'.

## Not to be confused with

- **pūrvakarma.** The preparatory phase (Su.Su.5.3; WHO ITA-9.2.4); paścātkarma is the after phase of the same triad.
- **pradhānakarma.** The main procedure (WHO ITA-9.2.68) that paścātkarma follows.
- **saṁsarjana krama.** The graded diet after vamana/virecana; often described as the paścātkarma of those procedures (Bhishagratna's footnote places 'rice meal' under paścāt-karma), but it is one content of the phase, not its name.
- **parihāra kāla / parihārya.** The period and the things to be avoided after a procedure (Ca.Si.1.4 asks 'किं वर्जनीयं प्रतिकर्मकाले, कृते कियान् वा परिहारकालः'); restrictions during paścātkarma rather than the phase itself.
- **paścāt (adverb) in other compounds.** NIA 2011 has only 'mūtrayutaḥ pāk paścāt' (a symptom description); the therapeutic compound is absent there.

## Renderings that do not hold

- post-operative care (as the whole meaning). Correct for Shalya, but Sushruta scopes the triad to all treatment of disease and WHO scopes it to panchakarma aftercare.
- follow-up (in the biomedical sense of review visits). Paścātkarma is an active regimen (diet, restrictions, further procedures), not monitoring.
- after-treatment. Suggests a lesser treatment given afterwards; the phase is part of the one karma that the triad divides.

## Open questions

Recorded rather than resolved.

- Whether Charaka uses the compound paścātkarma; the Siddhisthana speaks of parihāra-kāla and saṁsarjana without this term in the lines read.
- The commentary (Dalhana) behind Bhishagratna's footnote assignments was not consulted.
- The edition behind siva.sh's verse numbering is unstated; wikisource's is Hari Sastri 1980.
