How it is translated, and whether the translation holds

Each rendering is given with the source that uses it. The assessment is ours.

Main therapeutic procedure

WHO International Standard Terminologies on Ayurveda (2022) · ITA-9.2.68; description: 'The main therapeutic procedures usually adopted in the context of panchakarma and surgery.' IAST given as pradhāna-karma · comparator-standard

Factual report of the WHO rendering. Note WHO scopes the term to both panchakarma and surgery, not surgery alone.

the Principal therapeutical or surgical appliances (Pradhana-karma)

Bhishagratna, Sushruta Samhita vol. 1 (1907), Sutrasthana ch. V · wisdomlib doc122892; Bhishagratna prints no Sanskrit verse numbers · public-domain-translation

Edwardian 'appliances' means measures/applications, not devices.

त्रिविधं कर्म- पूर्वकर्म, प्रधानकर्म, पश्चात्कर्मेति; तद्व्याधिं प्रत्युपदेक्ष्यामः ॥३॥

tr. Sanskrit text (no translator) · source

The threefold division of therapeutic action into pūrvakarma, pradhānakarma and paścātkarma, stated as applying 'with reference to each disease'.

Sushruta SamhitaSutrasthana V (Preliminary measures / Agropaharaniya):Not numbered by Bhishagratna; corresponds to the sentence numbered 3 on siva.sh (see previous entry). Do not cite as 'Su.Su. 5.3 (Bhishagratna)'. · checked against the source

The entire course of medical treatment in connection with a disease may be grouped under three subheads, as the Preliminary measures (Purva-karma); the Principal therapeutical or surgical appliances (Pradhana-karma); and the After-measures (Paschat-karma).

tr. Kaviraj Kunja Lal Bhishagratna (1907) · source

Bhishagratna's English for the three terms; he frames them as covering 'the entire course of medical treatment', not surgery only.

Not to be confused with

pūrvakarma
The preparatory phase (e.g. snehana, svedana before panchakarma; pre-operative preparation in surgery); pradhānakarma is the main act itself.
paścātkarma
After-measures (e.g. saṁsarjana krama, wound care); frequently lumped with pradhānakarma in loose descriptions of 'the procedure'.
pañcakarma
Pañcakarma names five specific pradhānakarmas; pradhānakarma is a phase label that applies to any procedure, surgical or purificatory.
pradhāna (as adjective, e.g. pradhāna marma)
'pradhāna' alone means chief/principal; only the compound with karma denotes the procedural phase.

Renderings that do not hold

  • main surgery / operation. Both Sushruta's framing and WHO scope the term to any treatment course including panchakarma; restricting it to surgery loses the panchakarma usage.
  • primary treatment / first-line treatment. Imports the biomedical sense of treatment hierarchy; pradhānakarma is a temporal phase, not a priority ranking.

Open questions

Recorded rather than resolved. Each one is something this entry cannot currently settle.

  • Which Sanskrit edition siva.sh follows for Su.Su. 5.3 (not stated on the page); verify against Trikamji/Acharya edition before publishing the verse number.
  • Whether Charaka uses the compound pradhānakarma at all; carakasamhitaonline Kalpana Siddhi page did not formally use the three terms (fetched 2026-10-07).
