METHOD: This study includes clinical trial studies that assessed the efficacy and/or safety of saffron in ADHD patients. Non-English papers, review articles, commentaries, letters, observational studies, thesis, animal studies, in-vitro studies, and conference abstracts were not included. The risk of bias in randomized studies was evaluated based on the Cochrane RoB.2, and risk of bias in pre-post intervention studies was assessed using the ROBINS-I tool.
RESULTS: Four studies met our inclusion criteria with a total of 118 patients. The results manifested an efficient role of saffron as either an adjuvant therapy to MPH or a single therapy against ADHD, without significant safety issues.

## Recent safety updates

- Doses up to 1.5 g/day considered safe. Toxic effects reported at 5 g. 10 g is abortive. 20 g is potentially lethal.
- Most clinical studies use 30-100 mg/day with good safety profile.
- CYP3A4 inhibition may increase toxicity of narrow therapeutic index drugs: digoxin, warfarin, cyclosporine A.
- Anticoagulant properties may increase bleeding risk when combined with warfarin, heparin, aspirin, or clopidogrel.
- May enhance effects of antidepressant medications - additive serotonergic effects require monitoring.
- Contraindicated in pregnancy due to uterine stimulant and abortifacient effects at high doses.
- Excessive consumption can cause dizziness, nausea, vomiting, uterine bleeding.
- LD50 values: 1.6 g/kg (stigma extract) and 6 g/kg (petal extract) in animal models.
- Saffron extract, safranal, and crocin have little or no acute toxicity at therapeutic doses.

**Standard Dosage:** 20-100 mg standardized extract per day (typical clinical dose 30 mg/day for mood support); 50-200 mg dried stigma in traditional use; do not exceed 1.5 g/day

**Bioavailability:** Crocin (water-soluble carotenoid glycoside) has moderate oral bioavailability (15-25%); hydrolyzed to crocetin in the GI tract, which is the primary absorbed form with 50-65% bioavailability. Safranal is volatile and rapidly absorbed. Bioavailability improved with phospholipid complexation (Phytosome technology) and nanoemulsion formulations. Sublingual delivery bypasses first-pass metabolism for safranal.

**Optimal Timing:** Morning dose for mood and cognitive benefits; with warm milk at bedtime per traditional use (Kesar-Doodh)

**Standardized Extract:** Standardized stigma extract (affron or equivalent), NLT 3.5% Lepticrosalides (crocins + safranal combined); or standardized to NLT 2% crocin and NLT 0.3% safranal by HPLC-DAD

**Shelf Life:** 24 months for standardized extract capsules in aluminum blisters; 12-18 months for raw stigma (whole); 36 months for properly sealed nitrogen-flushed capsules

**Storage:** Store at 15-25 deg C in airtight, light-protected containers (amber glass or aluminum blisters). Nitrogen atmosphere recommended. RH <40%. Whole stigmas in sealed tins away from light, heat, and moisture.
