(2024) is used here only for classification: it states that in Ayurveda "Rasa Shastra includes the fabrication of Bhasmas and Pishti" and that "A limited number of studies have been carried out using Ayurvedic preparations derived from gems or precious stones that were used traditionally." That paper characterised Badarashma Pishti (Lapis judaicus), not a sapphire preparation, and reported calcite, quartz and aragonite with Ca, Mg, O and Si by EDAX; although its methods list "identification of heavy metals", the abstract reports no heavy-metal result, so no metal statement is taken from it. Saper et al. (2008) measured metals by x-ray fluorescence in Ayurvedic medicines bought over the Internet: 193 of the 230 requested medicines were received and analysed, and the prevalence of products with detectable lead, mercury or arsenic was 20.7% (95% CI, 15.2%-27.1%). Neither origin nor a manufacturing claim discriminated: US-manufactured prevalence was 21.7% (95% CI, 14.6%-30.4%) against 19.5% (95% CI, 11.3%-30.1%) for Indian products (P = .86), and among the metal-containing products 95% were sold by US web sites and 75% claimed Good Manufacturing Practices. Rasa shastra products had a greater prevalence of metals than non-rasa shastra products (40.6% vs 17.1%; P = .007) and higher median concentrations of lead (11.5 microg/g vs 7.0 microg/g; P = .03) and mercury (20,800 microg/g vs 34.5 microg/g; P = .04). The authors state that "All metal-containing products exceeded 1 or more standards for acceptable daily intake of toxic metals." Saper et al. (2004) found that 14 (20%) of 70 Ayurvedic herbal medicine products bought in Boston-area South Asian stores (95% CI, 11%-31%) contained heavy metals: lead (n = 13; median 40 microg/g; range 5-37,000), mercury (n = 6; median 20,225 microg/g; range 28-104,000) and/or arsenic (n = 6; median 430 microg/g; range 37-8130). They conclude that "If taken as recommended by the manufacturers, each of these 14 could result in heavy metal intakes above published regulatory standards", that users "may be at risk for heavy metal toxicity", and that "testing of Ayurvedic HMPs for toxic heavy metals should be mandatory." Mikulski et al. (2017, epub 2018) analysed 252 samples of Ayurvedic products and herbal supplements collected during a public health lead-poisoning cluster investigation. Lead was detected in 164 samples (65.1%), median 4.9 mg/kg, range 0.46-43,200 mg/kg; mercury in 97 (38.5%), median 53.0 mg/kg, range 0.8-279,000 mg/kg; arsenic in 82 (32.5%), median 8.4 mg/kg, range 1.0-44,800 mg/kg. The exceedance figures are taken with the authors' own denominators: "Almost half of samples containing mercury, 36% of samples containing lead and 39% of samples containing arsenic had concentrations of those metals per pill that exceeded, up to several thousand times, the recommended daily intake values for pharmaceutical impurities", which in Table 2 is 59 of the 164 lead-containing samples (35.8%), 48 of the 97 mercury-containing samples (49.5%) and 32 of the 82 arsenic-containing samples (39.5%) exceeding the USP oral permissible daily exposure per pill for a 50 kg person, assuming a 500 mg pill and one pill per day. The same paper documents lot-to-lot variability within one product family: Indukantham tablets from the same provider ranged from 43 to 43,200 mg/kg lead and 13 to 950 mg/kg mercury. The authors state a limitation that bears directly on interpretation: "This study did not conduct metals speciation analysis of the Ayurvedic products", so the chemical species present, and therefore the bioavailability behind those concentrations, are not established. Hore et al. (2022) reported on 584 samples of health remedies collected by the New York City Department of Health and Mental Hygiene during poisoning investigations and store surveys between 2010 and 2019, finding a significant association between blood lead levels and estimated cumulative daily lead exposures among adult users of rasa shastra Ayurvedic medications, and that average blood lead levels among adult rasa shastra users were significantly higher than levels among those using other types of non-Ayurvedic health remedies. FDA warns that "using unapproved ayurvedic products containing harmful levels of heavy metals may cause heavy metal poisoning" and names lead, mercury, arsenic, iron and zinc among the metals such products may contain. One apparent counterpoint at pishti class level does not transfer: Joshi et al. (2020) reported for Trinakantamani (Amber) Pishti that "All heavy metals in the samples were within the permissible limit." Three facts void any transfer of that reassurance to a marketed gem pishti. The three samples were prepared by the investigators themselves using the standard reference method rather than bought from the market, so the finding says nothing about market supply; Trinakantamani is amber, a fossilised resin whose chief elements the same paper found to be carbon, hydrogen and oxygen, so it is not a metal-bearing gem or mineral; and it is a different preparation from the one this record concerns.
