## Recent safety updates

- Most common adverse effect: mild gastrointestinal discomfort (stomach upset, diarrhea, bloating) following oral consumption.
- Hypoglycemia risk: Due to possible hypoglycaemic effect, close monitoring of glycaemic control recommended in diabetic patients (EMA monograph warning).
- Allergic reactions: 10 case reports/series documented allergic reactions or hypersensitivity (cross-reactivity possible with chickpeas and other legumes).
- Maple syrup odor: Characteristic odor in sweat, urine, or skin of mothers and breastfed infants -- can mimic maple syrup urine disease in neonatal screening.
- Suspected hypokalemia reported in some cases.
- Pregnancy contraindicated: Not recommended during pregnancy due to potential uterotonic effects. Unknown if it passes into breast milk.
- No known severe, serious, or moderate drug interactions documented, but caution with diabetes, asthma, and [potassium](/glossary/compounds-o-q/#potassium)-lowering medications.
- Hepatotoxicity: No clinically apparent liver injury cases despite widespread use; prospective studies show no effect on serum enzyme levels at therapeutic doses.
- Case report (2025): Ribociclib-induced hepatotoxicity potentially exacerbated by concurrent fenugreek supplement use, suggesting possible CYP-mediated interactions with oncology drugs.
- No fatalities attributed to fenugreek use in the literature.

## Dosage forms and preparation

**Dosage Forms:** Churna (seed powder), Capsules, Tablets, Decoction, Sprouted seeds, Extract (hydroalcoholic), [Mucilage](/glossary/compounds-l-o/#mucilage) gel, Topical paste/hair mask

**Standard Dosage:** 5-30 g seed powder per day (diabetes management); 500-1000 mg standardized extract twice daily; 5-10 g soaked/sprouted seeds daily

**Bioavailability:** 4-Hydroxyisoleucine has good oral bioavailability (~60%). Diosgenin is lipophilic with moderate absorption enhanced by lipid co-administration. Galactomannan fiber (50% of seed) is not absorbed but slows glucose and lipid absorption, providing therapeutic benefit. Trigonelline has good water solubility and oral bioavailability (~70%). Steroidal saponins undergo GI hydrolysis before absorption.

**Optimal Timing:** 30 minutes before meals for glycemic control (fiber and 4-hydroxyisoleucine need time to form gel); soaked seeds in morning for general health; topical paste overnight for hair health

**Standardized Extract:** Standardized seed extract (10:1) with minimum 40% galactomannan and 4% steroidal saponins (as diosgenin equivalents). 4-Hydroxyisoleucine-enriched extract (minimum 20% 4-HI) for targeted glycemic products. Defatted seed powder for fiber supplementation. Diosgenin-rich extract (minimum 10%) for hormonal applications.

**Shelf Life:** Whole seeds: 36 months. Ground powder: 18 months. Extract capsules: 24 months. Mucilage gel: 12 months with preservative.

**Storage:** Whole seeds in cool, dry conditions at room temperature. Ground powder in airtight containers below 25 deg C. Extract capsules in blister packs with desiccant. Mucilage gel refrigerated at 2-8 deg C after opening.
