## Recent safety updates

- Topical curcumin is generally well-tolerated with minimal adverse effects; may cause temporary yellow staining of skin. The 0.5% microemulgel formulation showed excellent tolerability in clinical trials with no significant side effects
- Major limitation is low oral bioavailability; enhanced formulations (nano-curcumin, liposomal curcumin, [piperine](/reference/piperine/) combinations) improve absorption. Oral curcumin at high doses (>8g/day) may cause GI disturbance. GRAS status by FDA for food use

## Dosage forms and preparation

**Dosage Forms:** Churna (powder), Tablet, Capsule, Kashayam (decoction), Lepa (paste for external use), Taila (oil), Soft gel capsule, Nano-formulation

**Standard Dosage:** 3-6g powder daily; 400-600mg curcuminoid extract twice daily; 15-30ml decoction

**Bioavailability:** Curcumin has notoriously poor oral bioavailability (<1%) due to low aqueous solubility, rapid metabolism, and poor absorption. Enhancement strategies: (1) Piperine co-administration increases bioavailability 20-fold; (2) Phospholipid complexation (Meriva/phytosome) improves absorption 29-fold; (3) Nanoparticle formulations (nanocurcumin); (4) Amorphous solid dispersions; (5) Self-emulsifying drug delivery systems (SEDDS); (6) Liposomal encapsulation; (7) Adjuvant use with fats/oils as per traditional practice (Haldi Doodh/golden milk).

**Optimal Timing:** With meals containing fat for improved absorption; traditionally taken with warm milk or ghee at bedtime

**Standardized Extract:** Curcuminoid extract standardized to 95% total curcuminoids (curcumin, demethoxycurcumin, bisdemethoxycurcumin); Turmerone-rich oil fraction (ar-turmerone >40%); Phytosomal curcumin (phosphatidylcholine complex)
