- Topical application is well-tolerated; adverse reactions in clinical trials with Berberis aristata root extract were minor and comparable to placebo group. Oral berberine may interact with CYP3A4-metabolized drugs

## Dosage forms and preparation

**Standard Dosage:** 3-6g powder twice daily; 500mg standardized extract capsule twice daily; 50-100ml decoction twice daily; eye drops as directed

**Bioavailability:** Primary active berberine has notoriously low oral bioavailability (< 5%) due to extensive first-pass metabolism by CYP [enzymes](/glossary/compounds-d-g/#enzymes), P-glycoprotein efflux, and poor aqueous solubility. Enhancement strategies: co-administration with [Pippali](/herb/pippali/) ([piperine](/reference/piperine/) inhibits P-gp and CYP3A4, increasing berberine bioavailability 2-5 fold); solid lipid nanoparticle formulations increase AUC by 4-fold; berberine-phospholipid complex (phytosome) improves absorption; cyclodextrin inclusion complexes enhance solubility. Self-nanoemulsifying systems (SNEDDS) show 6-fold improvement in preclinical studies.

**Optimal Timing:** Before meals with warm water for metabolic conditions; after meals for GI conditions to reduce mucosal irritation

**Standardized Extract:** Standardized to NLT 5% berberine by HPLC (dried extract); concentrated extracts available at 85-97% berberine HCl. Extract ratio 10:1 to 20:1 typical for capsule formulations.

**Shelf Life:** 2 years (powder); 3 years (tablet/capsule); 1 year (eye drops after opening); 2 years (eye drops unopened)

**Storage:** Store in airtight, amber or opaque containers below 25 deg C, protected from light. Berberine degrades under UV exposure. Eye drops: store at 2-8 deg C, discard 28 days after opening.
