## Where is it described in the classical texts?

Sahasrayogam (Taila Prakarana). A signature Kerala Ayurvedic formulation for Vata-Kapha predominant pain and inflammation. Contains 9+ herbs with tamarind juice and curd as special adjuvants.

## How does it work?

- COX-2 enzyme inhibition by oil extract constituents leads to reduced prostaglandin formation and localized anti-inflammatory action at joint tissues
- [Piperine](/reference/piperine/) from [Pippali](/herb/pippali/) (long pepper) acts as a bioenhancer, increasing transdermal absorption of other active constituents by up to 40% through disruption of keratinocyte lipid bilayer
- Gingerols and shogaols from dry [ginger](/herb/ginger/) inhibit leukotriene biosynthesis and TNF-alpha production, providing dual anti-inflammatory and [analgesic](/reference/analgesic/) effects
- Tamarind juice (acidic medium) and curd (lactic acid) lower the pH of the formulation, enhancing solubility and absorption of alkaloid compounds through the skin

## Which traditional uses are supported by research?

- Arthritis and joint pain management - validated by pilot study (2020) showing 65% of OA patients experiencing pain reduction and improved flexibility
- Neuromuscular pain including spondylosis and sciatica - supported by RCT (2021) demonstrating 25% additional pain relief over physiotherapy alone
- Tennis elbow and sports injury management - supported by pharmacological evidence of COX-2 inhibition and enhanced transdermal delivery of anti-inflammatory compounds

## What do recent clinical trials show?

*3 further claims previously listed here could not be traced to a published paper and have been removed. Classical formulations are researched largely in journals that PubMed does not index, so an absence here reflects the reach of the index rather than the state of the evidence.*

## Recent safety updates

- Generally well-tolerated externally. May cause warming sensation or mild redness in sensitive individuals due to [ginger](/herb/ginger/) and garlic content. Avoid application on open wounds, cuts, or broken skin. Patch test recommended before first use.
- Not recommended for individuals with garlic or mustard allergies. Large-scale double-blind trials are lacking, particularly for diabetic neuropathy and fibromyalgia applications. Avoid in acute pitta-inflammatory skin conditions. Keep away from eyes and mucous membranes.

## What is it made of?

### Mineral/Elemental Profile

- **Primary component:** Mineral-derived preparation
- **Note:** Composition varies by specific preparation method

**Analytical Methods:** XRD, ICP-OES, SEM-EDS

## Dosage forms and preparation

**Dosage Forms:** Thailam (medicated oil for external use), Cream (modern), Gel (modern)

**Standard Dosage:** External application: sufficient quantity for local massage, 1-2 times daily, as per Sahasrayogam

**Bioavailability:** Oil base provides excellent transdermal delivery of [costunolide](/glossary/compounds-c-d/#costunolide) (sesquiterpene lactone) and [gingerol](/glossary/compounds-d-g/#gingerol)/shogaol derivatives. Counter-irritant effect of [ginger](/herb/ginger/) compounds increases local blood flow enhancing drug penetration. Massage application further enhances absorption through mechanical action.

**Optimal Timing:** Morning and/or evening for joint pain, arthritis, and musculoskeletal conditions. Warm oil application preferred.

**Shelf Life:** 3 years from date of manufacture as per ASU guidelines for Taila

**Storage:** Store in amber glass or HDPE bottles in a cool, dry place. Temperature not exceeding 25°C. Protect from light.

**Marker Compounds:** Costunolide, Dehydrocostuslactone, 6-Shogaol, 6-Gingerol, [Sesamin](/glossary/compounds-q-t/#sesamin), [Cedrene](/glossary/compounds-c-d/#cedrene) (from [Devadaru](/herb/devadaru/))

**Quality Parameters:** Acid value (<5), peroxide value (<10), saponification value, refractive index, specific gravity, costunolide content by HPLC, 6-shogaol content (from dried [ginger](/herb/ginger/)), total sesquiterpene lactones, rancidity test negative, HPTLC fingerprint, microbial limits, heavy metals

**Vehicle (Anupana):** Not applicable — external use only. Warm oil for massage.
