## Key facts

- **Pharmacopoeia status:** Listed in Kerala Ayurveda traditional formulary; referenced in Bhaishajya Ratnavali and Sharangadhara Samhita; available from GMP-certified Ayurvedic manufacturers (Kottakkal, AVP, Dabur)

## Names and identification

## Where is it described in the classical texts?

Bhaishajya Ratnavali; Sharangadhara Samhita; Kerala Ayurveda tradition (medieval period, 8th-12th century CE); various classical compendia for Atisara and Grahani

## How does it work?

- Bilwa (Aegle marmelos) provides potent anti-diarrheal action through tannin-mediated astringent effect on intestinal mucosa
- Musta (Cyperus rotundus) provides Grahi (absorbent) and Deepana (digestive stimulant) properties, normalizing stool consistency
- [Trikatu](/herb/trikatu/) stimulates Agni (digestive fire), correcting Mandagni (weak digestion) that underlies IBS pathogenesis in Ayurveda
- Carminative spices ([cumin](/herb/cumin/), [coriander](/herb/coriander/), cardamom, caraway) reduce intestinal gas, bloating, and abdominal spasm
- Jaggery base provides prebiotic support and mild [laxative](/reference/laxative/) action, balancing the astringent herbs for normalized bowel function

## Which traditional uses are supported by research?

- Grahani (IBS): clinical trial validates 67-100% improvement across all IBS symptoms including pain, bloating, diarrhea, and mucus passage
- Atisara (diarrhea): anti-diarrheal action of Bilwa validated pharmacologically through astringent and [antimicrobial](/reference/antimicrobial/) mechanisms
- Pravahika (dysentery): traditional use supported by antimicrobial and intestinal mucosal protective properties of Bael
- Agnimandya (digestive weakness): [Trikatu](/herb/trikatu/) and carminative spices confirmed to enhance digestive enzyme activity and reduce flatulence

## What do recent clinical trials show?

- Tiwari R, Pandya DH, Baghel MS 2013. [Clinical evaluation of Bilvadileha in the management of irritable bowel syndrome](https://pubmed.ncbi.nlm.nih.gov/24696573/). *Ayu*. PMID [24696573](https://pubmed.ncbi.nlm.nih.gov/24696573/) · [doi:10.4103/0974-8520.127717](https://doi.org/10.4103/0974-8520.127717)
46 IBS patients treated for 12 weeks showed 67.09% improvement in abdominal pain, 100% resolution of bloating, 86.96% improvement in IBS-D, 100% resolution of mucus passage, and 97.50% improvement in urgency

## Recent safety updates

- Clinical trial in 46 IBS patients over 12 weeks showed no serious adverse effects at recommended doses
- Excess consumption may cause burning sensation, hot flashes, heartburn, or constipation; dose-dependent effects
- Some reports of itching and burning sensation may be attributable to low-quality brands rather than the formulation itself

## What is it made of?

### Key Active Markers

- Derived from constituent herbs (see individual herb profiles)
- Standardized on primary bioactive markers

**Analytical Methods:** HPLC fingerprinting, TLC (identity), LC-MS/MS (marker quantification)

## Dosage forms and preparation

**Dosage Forms:** Avaleha/Leha (semi-solid confection), Granules (modern), Capsules (modern)

**Standard Dosage:** 6-12 g twice daily, as per AFI Part I
